Licensed Practical Nurse (LPN) – Remote Care Coordinator

Licensed Practical Nurse (LPN) – Remote Care Coordinator
Full-Time | 100% Remote | Monday–Friday, 9:00 AM–5:00 PM | $23.00/hour

Ready to leave bedside nursing behind without leaving the profession?

If you're an LPN who's done with 12-hour shifts, physical strain, and being pulled in ten directions on the floor, this role gives you your evenings and your body back. We're a growing Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) company supporting nursing home patients, and we're looking for an LPN who wants meaningful clinical work from home.

What the day actually looks like:
This role focuses on data review and care coordination. You'll spend most of your time reviewing RPM data (vitals, trends, alerts) and coordinating care plans, with documentation and EHR work rounding out your day.

What you'll do:

  • Review daily RPM data and escalate critical alerts or concerns to providers promptly
  • Support accurate CCM, RPM, and BHI documentation for Medicare/Medicaid billing compliance
  • Build and adjust individualized care plans for long-term and acute care patients
  • Maintain meticulous EHR documentation for every patient interaction and data review

What you need:

  • Active, unrestricted LPN license issued by or held in FL, TX, PA, NY, GA, or OH (Compact license preferred, but not required)
  • Reliable high-speed internet and a private, HIPAA-compliant home workspace
  • Ability to work independently with minimal supervision
  • Basic computer literacy (comfortable navigating software, email, and typing efficiently)

Preferred:

  • Experience working in a Skilled Nursing Facility (SNF)
  • Experience with EHR/documentation systems
  • Familiarity with CCM, RPM, or BHI workflows in long-term or acute care
  • Strong written/verbal communication and sharp attention to detail

Why this role:

  • Trade the physical toll of bedside nursing for stable, data-focused clinical work
  • 100% remote, predictable Monday–Friday daytime schedule
  • Data-driven role
  • Ongoing clinical training and growth opportunities
  • Real impact on vulnerable patients, without the floor burnout

Compensation: $23.00/hour, full-time, Monday–Friday, 9 AM–5 PM

Pay: $23.00 per hour

Expected hours: 40.0 per week

Application Question(s):

  • Are you located in Florida, Texas, Pennsylvania, New York, Ohio, Washington, D.C., or Maryland?
  • Do you have a compact license?
  • Do you have PCC experience?
