Part-Time Nurse Practitioner – Telehealth / Whole-Person Care with Integrated Team

Albuquerque New Mexico

Scope of work
We are looking for a part-time Nurse Practitioner to serve as the medical clinician within a multidisciplinary behavioral-health and whole-person care model.

The role is primarily telehealth-based and focused on patients with chronic medical and behavioral-health needs. It is not a traditional full primary-care panel role.

Responsibilities will include:

  • Conducting initial telehealth medical evaluations and periodic follow-up visits
  • Reviewing medical history, medications, chronic conditions, and care plans
  • Managing common chronic medical conditions within the NP’s scope
  • Providing a limited amount of general primary-care support
  • Prescribing medications and ordering labs, tests, or referrals when clinically appropriate
  • Reviewing clinically significant findings identified through ongoing patient monitoring
  • Responding to issues escalated by the care team
  • Providing medical input and oversight for patient care plans
  • Coordinating with behavioral-health clinicians, primary-care providers, specialists, and other members of the patient’s care team as needed

Team-based model
Theraputech’s LCSWs and other care-team staff will perform much of the ongoing monitoring, patient outreach, between-visit support, documentation, and care coordination.

The NP will primarily focus on work requiring an advanced-practice medical clinician: medical evaluation, clinical decision-making, chronic-condition management/oversight, medication and treatment decisions, review of significant findings, and escalation when a patient’s needs require NP involvement.

Weekly hours / schedule
Approximately 5–10 hours per week initially.

The schedule can be flexible and the work will be primarily remote/telehealth. Hours may grow over time as patient volume increases.

Compensation
Approximately $55–$75/hour as a 1099 independent contractor, depending on experience and fit.

Ideal background

  • Active New Mexico NP license
  • Medicaid credentialed preferred
  • Comfortable managing common chronic medical conditions
  • Experience with telehealth and/or care management preferred
  • Comfortable working with behavioral-health populations
  • Strong interest in collaborative, multidisciplinary care
  • Comfortable working alongside LCSWs and other care-management staff rather than operating as a traditional standalone primary-care provider

Pay: $55.00 - $75.00 per hour

Expected hours: 5.0 – 10.0 per week

Work Location: Remote
